Balanced lips, naturally: lip asymmetry treatment in Massapequa
One side of your lip fuller than the other, a peak that sits higher, or a smile that pulls unevenly. Sometimes it's natural, sometimes it's left over from past filler. At Skin Space in Massapequa, we help clients across Long Island bring the lips into better balance with precise, carefully measured treatment.
Also known as lopsided lips, lip asymmetry, uneven lip filler and an uneven smile.

Why lips can be uneven
A little asymmetry is normal; some is worth correcting.
No one's lips are perfectly symmetrical. Small differences between the two sides, or between the upper and lower lip, are part of every natural face. But when one side is noticeably fuller, a peak sits higher, or the lip pulls to one side when you talk or smile, it can be all you see in photos.
Uneven lips have different causes, and the cause guides the treatment. Volume differences respond well to small amounts of filler. Movement differences may respond better to a lip flip. And when the unevenness comes from old filler that has settled or moved, dissolving it first is often the best way to start fresh.
Volume differences
One side of the upper or lower lip is fuller or flatter than the other.
Shape and border differences
Uneven cupid's bow peaks or a lip border that's sharper on one side.
Movement differences
A lip that lifts or pulls more on one side when you smile or talk.
If both lips are thin overall, see thin lips and lost lip volume. For broader differences between the two sides of the face, see facial asymmetry and balance.
What causes it
Some differences are built in; others develop.
Lip asymmetry can be present from birth or develop over time.
- Natural development, since the two sides of the face rarely grow identically.
- Muscle movement, with one side of the mouth more active when you talk or smile.
- Previous filler that was placed unevenly or has moved over time.
- Injury, scarring or surgery on or near the lips.
- Dental changes, such as missing teeth or bite differences that change lip support.
- Habits, like chewing on one side or biting one side of the lip.
Lip balancing treatments at Skin Space
Your Skin Space provider will measure and compare both sides at rest and in motion before treating.
-
Dermal Filler
Small, precise amounts of soft lip filler add volume or definition where one side has less, bringing both sides into closer balance.

- Downtime
- Swelling and possible bruising for a few days

Lip Flip
A small dose of wrinkle relaxer placed unevenly, if needed, can soften a stronger pull on one side or roll a thinner upper lip outward for a more balanced look.

- Format
- A quick appointment, repeated every few months
- Downtime
- None
- Good to know

Filler Dissolving
An enzyme breaks down hyaluronic acid filler that has settled unevenly or moved, so the lips can be reassessed and rebalanced from a fresh start.

- Format
- One appointment, sometimes followed by new filler after a waiting period
- Downtime
- Mild swelling for a day or two

What to expect at Skin Space
Precision matters most with lips.
Detailed assessment
We look at your lips at rest, when you talk and when you smile, and often take photos to compare both sides.
Your plan
If you have old filler, we may recommend dissolving it first. Otherwise, your provider plans small, targeted adjustments.
Treatment
Filler is added gradually with numbing for comfort, checking balance as we go.
Review
We check your result after about two weeks, once swelling has settled, and make fine adjustments if needed.
Caring for your lips at home
Simple care for the best result.
- Expect uneven swelling at first. Lips often swell more on one side for a few days, so wait two weeks before judging symmetry.
- Use ice wrapped in a cloth for short periods to reduce swelling.
- Avoid lipstick, alcohol and strenuous exercise for 24 hours after treatment.
- Try not to bite or chew on one side of your lip.
- Keep lips moisturized and use a balm with SPF outdoors.
When to check with your doctor
New lip asymmetry can be a warning sign.
Get medical care if you notice:
- Sudden drooping of one side of the mouth, especially with slurred speech or arm weakness. Call 911 immediately
- New weakness or numbness in the lip or face, even without other symptoms
- A new lump, sore or color change on the lip that doesn't heal
- Swelling, pain or hardness in lips that have had filler before
- Asymmetry related to a cleft lip repair or significant scar, which may need a surgeon's input
After any filler treatment, get emergency care right away for severe pain or skin that turns pale, white or dusky. Every Skin Space consultation includes a review of your health history so we can choose treatments that are safe for you.
Common questions
Is it normal for my lips to be uneven?
Yes. Everyone has some lip asymmetry. Treatment focuses on differences that bother you, not on making both sides identical.
Can filler fix uneven lips?
Often, yes. Small amounts of filler can balance volume and shape. If the difference is from movement, a lip flip may work better.
My lip filler from somewhere else looks uneven. Can you help?
Usually, yes. Hyaluronic acid filler can be dissolved, and once the lips have settled, we can rebalance them.
Why does one side swell more after filler?
Uneven swelling in the first few days is common. Wait about two weeks before judging the final result.
How long does it take to see the final result?
About two weeks for filler, once swelling settles, and about a week for a lip flip to take effect.
